We haven't conducted a disability audit of the library, but the University has recently used a firm of consultants to do a disability audit across campus. The firm was called DisabledGo (www.disabledgo.com).

We are going to be carrying out a full disability audit of the library,
covering all services and functions
I would be interested to know if anyone else has done something similar.
If anyone has I would be interested to know how they went about it: who
was involved - did you use external consultants (and if so who) - did
you use any other guidance such as checklists.
