If you haven't already done so-I would refer them to an OHP for starters before going down the route of psychologists otherwise you could be making mountains out of molehills. I have up most respect for clinical psychologists ECT: however you will be receiving an extremely long & detailed report from them which may just complicate matters (I have had experience of this)
Is this person willing to consent to seeing a psychologist- if not then?? to be asked
Identify if this person has a mental health issue. It is important not to "medicalise" a performance issue. Stick to the facts. I have had experience of such cases where HR have been pressing OH for information which simply does not exist about an individual instead of managing it themselves.
We are all rambling on here (well I am anyway) I would be interested to hear your story in more detail

Get a clin psych or else a work psych with counselling skills: one might
end up referring you to the other of course. Go the BPS.org.uk and
look among the chartered psychologists in NE London/Essex.
