      smurf: propagate quality properly when concatenating
      
      smf_concat_smfGroup used to look at the smfData from smf_open_file
      to decide whether the input file had quality to copy when
      concatenating. This test fails badly for raw data because
      smf_open_and_flatfield creates a quality and populates it.
      
      Now decide on quality by looking at the smfData post flatfielding.

      Over the years the code evolved into
      
        for pass = 1 to 2
          for j = firstpiece to lastpiece
      
           if (pass == 1 ) {
             initial sanity checks
           }
           if (pass = 2 ) {
             read data
           }
         }
        }
      
      so I changed it to the easier to realise that nothing clever is happening
      
        for j= firstpiece to lastpiece
           initial sanity checks
        }
        for j = firstpiece to lastpiece
           read data
        }
      
      Indenting changed so the patch is bigger than it really is.

      smurf: Make MAKECUBE SPECBOUNDS parameter consistent with FLBND and FUBND
      
      Changed specbounds values to be the spectral values at the outer
      edges of the first and last channel, rather than the values at
      the channel centres. This makes it consistent with the values
      written to the FLBND and FUBND parameters.
