We use an external survey (an in-house php/mysql solution). The link in
all Bb areas passed the module code and username for tracking and
analysis. We have the link in our template modules so it is in all new
areas (we did have to do some boring manual work to copy it into all
existing areas when we wet it up).
This lets us look at stats across years, across all modules on a course
etc.

I have had a colleague recently that uploaded a survey / test (user
error) for all staff / students, in a support area that the whole
colleague uses.
Just one problem, opening the Grade Centre. It crashes a lot and brings
up script errors, but it will eventually open. So I guess it depends how
many users you have (we have about 14,000+ on the system)
But if you wanted to pool the results to one area still, you may still
come across the same problem anyway on Blackboard. Also would you manage
to get all the results in from tutors if you left it in their hands??
If it was me I would have an external survey, and just include a link in
an area the college/university could see.
