We have a policy on the Date of an item, to which subsequent messages can be
re-dated. But at some point e.g. in a multi-specialty journey of care, a
new set of items deserves their own date.
Perhaps Date is the least ambiguous of the metadata options; and all systems
can sort/display by it.

You can scan a batch of documents under an individual problem header.
They will each be spearate attachments.
