You could try Arnold-Forster, F. Studies in Church Dedications or
England’s Patron Saints, 3 vols., London, (Skeffington & Son, 1899). This
lists church dedications rather than things like altars or chapels, but it
is useful. You could also contact Dr Graham Jones at St John's Oxford
([log in to unmask]), who knows more than anyone else about
dedications.
