Andy,
Don't know of any 'Idiots guide' to ICD's. I have found the WHO's
site info tends to be a little difficult and not much help. It really
is a matter of matching what you think you have with what you have on
the IDC's. The list does however lend itself mostly to primary care
presentations. There is a seperate coding for injury.
I have an ICD10 list on Excel spreadsheet at work. I will get it to
you tomorrow. Being on spreadsheet, the windows search facility makes
usage easy, but interpretation can be difficult. Not everything is
covered so this too adds to statistical variabilities, if that is what
you plan to use it for.
