Role Transition and the Nurse Practitioner: An Investigation into the Experience of Professional Autonomy

Abstract
Amongst specialist nursing roles, the position of the nurse practitioner is of particular interest because nurse practitioners have evolved within an interprofessional philosophy of care, and have complex issues relating to the scope of their professional practice, their individual and professional identity, and their professional autonomy. This paper summarises a study that explored nurse practitioners' experiences of role transition, professional autonomy, and the boundaries of professional practice.
The research was conducted using a phenomenological hermeneutic approach inspired by Paul Ricoeur. Narrative interviews with fourteen nurse practitioners were transcribed to create a series of texts for subsequent analysis, a process which developed both a descriptive analysis and an interpretive account of the participants' experiences.
The findings reflected a generally positive view of practice on the part of nurse practitioners, but highlighted a number of barriers to the development of their professional autonomy, and reinforced the need for clearer definition of the nurse practitioner role. The study recommended that further reflection on the nature of advanced practice nursing, and on the relationship between nurse practitioners and other health care workers might help to realise the potential of the nurse practitioner.
Biography
Andy Mercer has held various roles within the University over the past 20 years. A mental health nurse by background, he is currently HSC's Professional Lead for Mental Health & Learning Disabilities. He is linked to mental health nursing practice via the University's partnership with Dorset Healthcare NHS Trust, and is involved in external work with the Nursing & Midwifery Council, most recently as part of the NMC working group developing new competencies for pre-registration mental health nursing. Andy completed his PhD at Bournemouth University in 2007.
