From Roger Ayers
It appears at the front of the 'Destroyers at Jutland' section of the first
English edition of Sea Warfare (1916) without the title and in the first
edition of The Years Between' (1919) and in subsequent collections with the
title.
There was US edition of Sea Warfare in 1917, which I do not have, so it
might possibly have acquired the title in that.
Hope this helps.

Subject: My Son Jack
The poem ‘My Boy Jack’ was first published, untitled, in the London Daily
Telegraph on 19 October 1916 to accompany the first of a group of Kipling’s
four articles on the Battle of Jutland collectively entitled ‘Destroyers at
Jutland’.
Is anyone able to tell me when the poem acquired its title?
Later, Kipling also added dates beneath the the title; one book/article on
Kipling gives these dates as 1914-1918, another 1911-1918. Is anyone able to
tell me what dates were added and when they were added?
Many thanks Brian Southam
