I suggest you look at the assessment criteria and brief guidance notes
that accompany each criterion, on the assessment form that my Board
colleagues will use to assess your application,

Each applicant is likely to bring different experience and knowledge to
their portfolio, but the assessors will need to be convinced that all of
the criteria have been met. The CDG's Chartership preparation courses
give excellent guidance on how to set about threading together your
evidence. Your mentor's experience is invaluable at this stage. Finally,
the national NHS LIS has a terrific support network.
A word of encouragement. Most applicants reaching the stage you're at
doubt that their evidence would make the grade and yet most make
successful applications first time. You might have to make a couple of
stabs at pulling together your information until you're happy with it
(again your mentor will offer helpful guidance). I suggest you measure
each item of evaluation and portfolio material against three related
questions - What? So what? What next? The combined answers to each set
of questions should provide professional reflection on your practice.

Hi everyone,
I'm currently in the process of trying to get my portfolio together,
sorting out the evidence etc, but I'm struggling to decide what should
go where. Looking at some example portfolios I've noticed that others
have split their evidence into 4 main categories:
Personal and Service Performance
Professional Development
Personal Progession
Wider Professional Context
However, I'm struggling to categorize all my evidence in this way. Can
anyone shed any light on the exact difference between "professional
development" and "personal progression", and "personal performance".
Does anyone know the types of evidence required for each category?
And does anyone know if you have to use these categories at all?
