This is a list of iPod/Phone apps for AAC(attached) from someone else from the US ALS-AT list, re-posted with permission.
As Andy said, volume could be an issue - a backbox amp from Smartbox, or some bluetooth speakers could help with this... Also as Aejaz noted, there are other handheld options around too.. Your patient sounds pretty 'complex', I imagine it might take quite a lot of tweaking to find just the right thing - have you local services that could support that?

I am going to suggest my patient (with tracheostomy, deafness, ataxia & limited visual field) try an ipod with an onscreen keyboard to type messages with her thumb/finger for others to read, and for her to read emails & text: it is small enough for her to hold a few inches from her nose....
Any thoughts/experience?
