Just some random thoughts on this........
Part M of Bldg Regs and BS8300 give an accessible toilet for independent use
as the minimum toilet provision for disable people. Unfortunately building
control officers usually have no alternative but to enforce Part M as read
and can not allow a toilet for assistive use as the main and only toilet in
a building.
As an access officer I have the 'luxury' of discretion and can consider
other options especially in existing buildings and can sometimes recommend
to building control that an alternative solution is a better solution given
all the circumstances. (not always though)
Pressalit Care produce equipment which is mounted on a track and can be re
positioned. Perhaps a solution could be to utilise this sort of equipment
so that a large basin can be positioned alongside the pan so that it can be
reached from the toilet and then moved out of the way by a helper if
assistive use of the facility is required. (the helper would have to move
the basin back again though).... this could add to the cost as well......

I have also just heard of a small organisation fitting a "changing place"
toilet, and being told by building control that this does not meet
regulations because there is no hand basin within reach of the pan. They
have been told they will have to fit another cubicle which meets the
regulations.
