I urge caution before anyone apportions blame. Earlier this year, I was
contacted by an engineering firm who reported that a delivery of tools
arrived which contained shredded paper as packaging but that some of it
had not been shredded properly (half a dozen pages hardly at all). Some
contained our logo, whilst others contained full letterheads. They kindly
sent these to me and it turned out the pages related to a service user
(social care).
After a thorough investigation followed involving our confidential waste
contractors, the firm who supplied the tools and our social care workers.
In the end it turned out the documents in question were copies which were
provided to the service user. She subsequently confirmed that she took
them to work (at the tool suppliers) to shred. The shredder in question
was totally inappropriate for sensitive material (it was a strip shredder,
not cross cut) and didn't even manage to shred the pages. To dispose of
their waste shredding, they used it to dispatch orders.
So it is not always easy to apportion blame!
