There's more information on different scoring methods and cut-offs for the GHQ here: http://www.gl-assessment.co.uk/health_and_psychology/resources/general_health_questionnaire/faqs.asp?css=1

Subject: Re: threshold scores for the GHQ-12
Hi Dally,
My understanding is there are a number of different scoring methods used
for the various forms of the GHQ (a summary of which is given in:
Measuring health: a guide to rating scales and questionnaires By Ian
McDowell, and you can view the relevant pages using Google books), and
likewise a number of different thresholds. If you're not looking for
caseness though, then I'm not clear why the cut-offs are relevant - if you
are comparing two groups then I'd suggest using the total scores as your
data will be richer than using a binary classification and frequency
stats. Or you could maybe do both to put any differences in the context of
the morbidity in your sample (i.e. finding differences may not be as
relevant if all scores fall below the normal threshold, as you'd just be
looking at fine gradings of 'normal').
Hope that helps,
