I find that when I am on form my average consultation time is 13 to 14
minutes. I now book my surgeries with 13.3 minute appointments, in
other words I set up standard EMIS 10 minute appointments but with a
gap after every third patient.
With this system I tend not to run too late, except when there is an
exceptionally time-consuming consultation.

We have a fairly low consulting rate (2.6 per patient per year), which
I like to think is because we mostly consult "in depth" and reduce the
need for the patient to return. We now have some younger doctors in
the practice who consult faster, and so far our consultation rate per
patient has not risen.
