We had one box but were concerned about security. In the end we needed a higher number of indexed pages than we had with our initial box, so that was the final justification to just go ahead and get a second box and run them separately, killing two birds with one stone.
It does mean you can't do 'combined' searches across both sites, but that wasn't a requirement for us. What we have done though add a link to external search for the same term as a link (we have done it as a tab). Thinking about it more, I reckon you could use a one-box module to do a more elegant integration. Or if your licence / number of pages allows it, have your 'intranet' search appliance also index the external site(s) - it will mean that the external site is being crawled twice and you'll have two copies of the index (one on each appliance) but it would deliver the more integrated search that you appear to be needing.
