Dear All,
I recently asked:
"Do you offer a "book a librarian" service, using a web form or other means?"
Thanks to Isla Kuhn, Cambridge University, who sent details of her "Information Clinic" one to one sessions - these are booked by email or phone, rather than by a webform. See http://www.medschl.cam.ac.uk/library/trainingdates.html. Isla was, I think, the only respondent, apart from people asking for a summary or saying they were thinking about doing this as well!
Here are some examples I found through G**gle but they may not be representative. I couldn't see any examples from the UK.
Malmo University

Is specifically to book a librarian for a "guided session of information searching", so the form asks for the search terms being used so far as well as a preliminary idea of what days and times would suit the enquirer. This is the service that alerted us to this idea, as we had a visitor recently from Malmo.
