Validity of indirect comparisons: Empirical evidence and conceptual framework
Abstract:
Adjusted indirect comparison and mixed treatment comparison have been increasingly used for evaluating relative effect of competing healthcare interventions. There is still disagreement about the validity of indirect approaches. Some consider findings from indirect comparisons as essentially observational evidence while others believe that adjusted indirect comparison take advantage of the strength of RCTs. In this presentation, we will first discuss empirical evidence on the validity of adjusted indirect comparison. The available empirical evidence was mainly from studies of simulations and those that compared results of indirect and direct comparisons. Then we will discuss a conceptual framework of basic assumptions concerning adjusted indirect and mixed treatment comparison, including homogeneity assumption for conventional meta-analysis, similarity assumption for adjusted indirect comparison, and consistency assumption for the combination of evidence from different sources. Finally, we will provide a summary of methodological problems in the use of indirect comparisons.
