Matt,
Same happened to me a while ago. It most likely means that the image you use
to build the mask on is in the NEUROLOGICAL convention and not the
RADIOLOGICAL, preferred by the FSL tools.
Here is a link on how to check your image's convention and how to rewrite it
to the RADIOLOGICAL convention if necessary, before you use the FSL tools on
it (like making a mask under fslview):
