In my last post as an OHA I was covering a contract where a large
proportion of the workforce were Asian and when I checked with them at PEM
re: immunisations, I was informed by the majority of them they were screened
for TB pre-visa/pre-arrival for the UK.

According to the speaker at the recent World TB Day in Perth, Aus, the
incidence of TB in Australian born persons is dropping. The main source of
new cases is among immigrants. They are nearly all screened pre-arrival and
are negative on x-ray and often have no history of TB contact.
50% of the new cases are cases of reactivated latent TB which become
symptomatic within 2 years of arrival in Australia (causes of reactivation
are unknown). The remaining 50% of cases develop, sometimes up to 20+ years
after arrival in Australia.
Giving immigrants BCG on arrival is not an option, as the Health Authorities
feel that it is largely ineffective in adults.
While cases of TB may be on the rise in UK I would suggest that it is from
the same source as in Aus. One of the doctors I spoke to at the Seminar Day
told me she had just returned from 4 years working in the UK and noted that
there is no pre-visa/pre-arrival medical screening for TB. (Does anyone know
whether that is true)
If that is truly the case, then screening prior to arrival seems to be the
way to go. I cannot remember the figures but Australia has probably one of
the lowest rates of TB in the world. So, they must be doing something right.
