How appalling that the culture of the NHS has been so undermined and
so systematically degraded that this disaster has occurred.
Would a criminal investigation be appropriate? If a doc causes death
by misadventure or negligence then a manslaughter charge would surely
follow. If a manager's policies and procedures result in harm leading
to death, which seems to be what has happened here, surely the same
legal principles should apply?
