I'd start with the principle in the first line of the description: "A
non-zero value indicates that a title begins with a definite or
indefinite article that is disregarded in sorting and filing
processes." In other words, the nonfiling characters indicator is
only non-zero when there's an article to be skipped. Once you've
determined that you have an article, you go on to count everything up
to the first filing character. This yields:
245 05 The "Pearl"
245 00 "Citizens of no mean city"
That said, any system that has a problem with the alternatives
probably needs some attention:
245 03 The "Pearl"
245 04 The "Pearl"
245 01 "Citizens of no mean city"
