Not sure what archive you've looked at but this topic has been rumbling on this list, lis-link and lis-copyright (and a couple of other lists) every couple of months since then. Perhaps the topic/subject line didn't specify Electronic signatures!
There are many libraries and colleagues who now collect electronic signatures at the point of request placement. However, few are willing to pop-up and say so. Plymouth is an exception. We do, and have been collecting an e-signature since 2006. I'll happily provide more information and answer questions off-list so as not to clutter inboxes again!

We are investigating going over to the Add Address service on Unicorn. I'd like
to find out whether many institutions are now using electronic signatures at
the time of request to enable articles to be sent directly from the British
Library to their email inbox.
Or do you still get a physical signature from the user before sending the
request to the British Library?
I have checked the LIS-ILL archive and as the last posting seems to be in
2006 I thought a fresh question on this topic would be OK.
