That happens in SPM all the time!
If you use SPM5 and are talking about structural MRI (and transformation
to MNI/ICBM152 standard space), use the SEGMENT module which will give
you a sn.mat file describing the transformation.
You can then apply the transformation via the NORMALIZATION module, and
will obtain a w*img which is your transformed image.
There is extensive context-sensitive help as well, in case this isn't
precisely what you want to do.
