It is not just the NHS, if you read Porter and Teisberg Redefining Health
Care (http://www.hbs.edu/rhc/) they review the issues with most systems in
the world and beautifully demonstrate the most rationed health service in
the world is in the USA where rationing occurs by use of cheap outmoded
treatments,(i.e. do a poor job so the patient gets sicker than they should
because that allows the opportunity of making a profit again) they further
demonstrate that if you want a cheap health service you have to treat the
patient with your best treatment first line almost whatever it costs and
then give the data of how to do it from the best units to those that should
be doing better. Their explanation for why there is no adoption of this
model is that the demonstration of the proven fact of health professions
being motivated more by making people better than by making a profit is
incompatible with the political classes view of the world. Personnally I
think this book should be required reading for anyone with or contemplating
a career in the NHS.

There is a general theme here, in that there are quite a few things cropping
up where the NHS line (often hiding behind the NICE official line)
recommends something which lags behind what I would call the "what would you
recommend for your family" test.
