Hi Marie
I would say yes if:-
a) the signature is hand-written by the individual making the request
b) you are as certain as you can be that the person making the request is who they say they are
Cheers

Subject: Distance Learners
Can someone give me some advice regarding distant learners and inter library
loans please?
Is a faxed request form from a borrower acceptable regarding the signature?
Thanks in advance,
Marie
