Kate, if your client hits the Details tab she will see the explanation
why there is such a difference, here it is below.
"attention"[MeSH Terms] OR "attention"[All Fields] OR "mindfulness"[All
Fields]
Pubmed automatically selects appropriate subject headings and adds them
to the search. If in Ovid your client uses Thesaurus Mapping and chooses
"Attention" as a subject then searches the word attention in any field
she'll see similar numbers.
Hope that makes sense

Subject: Re: Searching Query: Ovid Medline V Pubmed
Dear Colleagues
Can anyone help with an Ovid Medline V Pubmed question?
A research colleague of mine has got the following odd results when
conducting her search and has asked me to look into it.
She searched the word 'mindfulness' in the following ways:
Ovid Medline (R) searching all fields using .af. = 424 hits
Pubmed basic search = 178722 hits
Pubmed Advanced searching all fields using the all fields tag = 364 hits
She knows Medline and Pubmed are different but was expecting the 178722
hits on Ovid. 424 was the most she could get and seems very low.
Any answers?
Thanks
Kate
