The article below is pretty old but quotes some response rates and cites
references for them.
In recruitment for volunteers from the community, anectdotally I've heard
the law of diminishing thirds used (1/3 of letters sent get a yes
response; 1/3 of those will be eligible after screening). I guess the
response rate to questionnaires will depend on the topic and any
incentives to respond.
