When you have a patient with a common name and no knowledge of their NHS
number, the name and address of the previous doctor - or the patient's
previous address if they had told their previous practice of previous
moves - helps to identify the patient on PDS.
I have this fear of either not receiving the record on GP2GP - or
incorporating the wrong record.
I'm not sure why you are so concerned - patients may have forgotten but
don't seem to mind.
