I'm just a student as well, but if your diagnosis is correct then I'm not
sure what the Alfredson's is doing for you.
My view is that those exercises will increase contraction of the
dorsiflexors which must already be contracted anyway to give the pain that
you are describing. In which case you need to be looking at something which
will relax the dorsiflexors and stretch them.
My suggestion would be to try massage over the affected area and transverse
friction massage if there is a localised spot that hurts.
Dorsiflexion stretching exercises will also help to align the fibres.
You may be better waiting until the physio gets back from holiday, though,
and find out what her treatment aims are before modifying the treatment they
have prescribed. As I've said, I'm just a student and may be talking out of
my sit upon!

I'm a physio undergrad & have been having Rx for pain in the
dorsiflexors/anterior compatments of my legs. The physio gave me Alfredson
eccentric plantarflexor exercises. I don't understand the rationale of
this Rx; I've looked at Alfredson's (& other) papers but they only discuss
the exercises in terms of Rx for Achilles tendinopathy.
Can someone explain the rationale hear, please. I did ask the physio to
explain, but she told me to look at the relevant papers & she's now on
holiday.
