Thanks for sharing this Paul
Your form looks good. I'd almost got to the point where your form is,
plus my nightworkers, that I think that I could add as an optional page
as only a small of our staff will be working nights.
Best wishes

Dear Carolynne,
I think adding questions for nuight work defeats the object of
shortening it to 3 questions. However where there are potential COSHH
implications of work or a statutory requriement for health risks these
can be added to the questionnaire.
I've attahced our form here.
Best wishes

Subject: [OCC-HEALTH] Pre-employment and HCWs: the 3 questions
I am in the process of reviewing my pre-employment health assesment
procedure for Healthcare Workers. In addtion to the '3 questions' I feel
that the specific questions for nightworkers should be included, where
the staff will be working nights. This is for baseline and to determine
suitabliity. Also to be included are questions about allergy and
dermatitis. Does anyone ask questions in relation to TB risk? Does
anyone feel that there is anything else relevant that should be included
for HCWs? Would be good to get some feedback on what any of you do for
your HCWs pre-emp within the NHS or elsewhere.
Thanks
Carolynne Davey
