Hmm, that's an interesting case I hadn't thought of!

The absence of a change in intellectual content from the addition of a
cover sheet would suggest there's no change in Expression, but I'm not
sure it's sufficient to conclude that it results only in the creation of
a new Copy/Item.
Might it not be the case that the addition of a cover sheet represents
the creation of a new Manifestation?
e.g. from FRBR 3.2.3

Where the production process involves a publisher, producer,
distributor, etc., and there are changes signaled in the product that
are related to publication, marketing, etc. (e.g., a change in
publisher, repackaging, etc.), the resulting product may be considered a
new manifestation.

So, could the addition of a cover sheet be considered "repackaging" by
the repository-service-as-"publisher"?
And if so
PDF without cover sheet = Manifestation 1; PDF with cover sheet =
Manifestation 2
(I'm not sure about this, BTW, just saying it seems to be an option)
