The handbook asks candidates to include a mentor completion form in their portfolio. This is to ensure that candidates have included evidence of their participation in the mentor scheme in their application.

Hi Claire,
I have recently sent my portfolio in and asked the Quals. department this
very question, as I couldn't find the answer anywhere. I was told by Lisa
Goldsmith that a copy had to be in each portfolio and an additional one
needed to be sent with the submission form for their mentoring files (4
copies in total).
I do think that it should be clearer in the documentation (online and in
the manual) that this is what is needed, as it is not at all clear what is
expected in terms of submitting this document.
