We've had long discussions about this. The cruxt of the matter is that
epidemiologically, TB is most likely in particular socioeconomic groups
and these groups are likely to be screened/ immmunised by the PCT.
Those not in identified risk groups/ localities will not be screened or
immunised. This should not necessarily be a great worry as the
likelihood of them developing TB and infecting patients/ coleagues is
low.
BCG efficacy is often quoted as being around 60% so those immunised with
BCG are not necessarily effectively protected and evidence of having had
BCG can be misleadingly stated as showing immunity.
For the cadets in clinical placements, the likelihood of acquiring TB
through their work is also very low. At this Trust, we are generally
only worried when a healthcare worker has spent prolonged time in a
single room with an infected patient or has perfromed chest physio or
mouth to muth resuscitation. It seems unlikely a cadet will be in this
situation.
If the cadet became a fully fledged student, they will probably be
screened by the OH provider to the educational institution. Naturally,
particularly vulnerable cadets may need screening due to their own
health i.e. any immunosuppressing conditions or treatments.
If you have concerns about individuals, NHS chest clinics can see
referrals or give telephone advice. They should not necessarily need a
referral by an NHS OH provider.

I simply can't understand why the secondary school system Jacqui,
I simply can't understand why the secondary school system of vaccination
has stopped, especially when there is a greater problem with TB, and
even UK hotspots.
I too have had a recent enquiry about a young adult getting TB
protection (Mantoux/BCG) before undertaking voluntary work overseas -
Isn't exposing these young people madness ?
Surely this just makes no sense, I am sure that some of the NHS Plus
sites may be able to help (we could - but logistics may prevent this),
but I agree with you regarding the potential influx, and the pressures
this could cause.

Hello
the college where i am based deals runs pre nursing courses (something
like a cadet scheme). The students are between 16 and 18 years old and
go on clinical placement within hospitals. We are expecting that this
years
students will not have been vaccinated against TB. They slip between
every net.
Their GPs wont offer TB screening/vaccination as it is an occupational
need.They are not employed. Their placement site wont offer vaccine as
they are not employed. The current service provider (an NHS plus site)
may not be able to continue with the service due to an anticipated
increase in their workload in their trust.they will still be able to t
spot test them but potentially wont be able to follow up with vaccines,
so i am looking
at alternate approaches.I know that ultimately this may be a service
that we have to offer however i want to explore all options.
Are there any companies out there who offer vaccination service (like
flu)
for TB?
