This is the one we use, which comes I think, from the OH Physicians. It
was supplied to us by ows anyway, and it is very effective.

good morning,
I have a query regarding a referral. I have a client who is off sick
with stress; they are currently under investigation for misconduct,
reported by colleagues and patients. I saw them yesterday and whilst
they are not fit for work, I am unsure about their fitness to attend for
a hearing. They are also under investigation into chest pains, which
they were admitted for, and is on cardiocor.
My concern is that they are lucid, able to plan taking up their case
with the HSE, but they do not want to attend for the hearing, they have
a Med 3 until end March. Is their a criteria to meet for fitness to
attend a hearing? I believe they they are lucid enough to attend, but
that they also have this chest pain related to stress, and will probably
have this pain 1) if put under extra pressure, and 2) because they do
not wish to attend.
