Hello,
It looks like your data is in neurological orientation,
there are some issues working with neurological data in the current
( and previous ) versions of FSL. If you convert your input data to
radiological ( http://www.fmrib.ox.ac.uk/fslfaq/#general_fslorient )
fast should work as expected...
