Due to the nature of the population profile of the London Borough of Brent
as well as the patient profile within our services there, race equality

Dear list members,
I am writing a practice-oriented briefing on the links between long-term
ill health, poverty and ethnicity. To make the briefing as useful as
possible, I am looking for examples of best practice in racial equality
in self-management programmes (including the Expert Patient Programme)
and in the delivery of welfare services to people with long-term health
conditions.
Does anybody have any examples of:
(1) best practice in including and working with minority ethnic groups
in self-management programmes, or in support groups for people with
diabetes, heart disease, arthritis or - in particular - mental
ill-health?
(2) self-management programmes or support groups that involve family
members as well as the individuals who are long-term sick, particularly
those working with minority ethnic groups? any that have been evaluated?
(3) best practice in the delivery of welfare services to minority ethnic
groups, such as providing specialist support with claiming benefits, or
support with job searching and getting back into work?
(4) I'm aware of some projects in which welfare support was being
provided in GP surgeries, to link up services and reach more
marginalised groups - could anybody provide any further details?
