Hi Diana,
My Trust is the largest in the country (or so I have been led to
believe!) and I am the nurse manager for the clinical team. The team
comprises 4 qualified OH specialist practitioners including me ( 2 Band
7 and 2 band 6), 6 OH advisors (Band 5) and 1 clinical support worker. 2
of the band 5 advisors are part time, as are 2 of the OHPs. We have
several large external contracts and plenty of smaller ones. I am also a
registered mentor and I work closely with the local university. I take
on a full time OH student every year who, although always supernumary,
does help with the workload!
With regard to TB screening, we invite potential new staff from
high-risk countries to come along pre-employment for Mantoux testing and
we complete a TB questionnaire. They return 48hrs later for the reading
of the Mantoux and we then inform the chest clinic that either the
person does or does not require follow up.
For new staff not from high risk countries, we have set Mantoux clinics
throughout the year (first Monday/Wednesday of each month) and we book
anyone who has not had a BCG into these clinics.
As always in OHS, clients often fail to attend for screening. In this
scenario, if the client DNAs twice, we inform management that we cannot
confirm their member of staff is free from infection.

We are in the same situation - the induction/training schedule at out
Trust does not give us any space until the staff get to the wards and
then it is impossible to catch them all in one go due partly to their
shifts and our lack of staff.
The problem seems to be getting the Trust to take this issue seriously
enough to make space for us right at the start. Then we have to find
the
staff.....not easy when there are only two full time equivalent nurses
to 3500 staff plus income generation work!!
I would be very interested to hear what others are doing about the TB
issue, and also what sort of staff numbers other NHS Trust occ health
depts have.
