Initially everyone who was put onto the database had to complete and sign
a Data Protection Form to confirm that they were aware that their data
would be available for all council staff to view, and for us to confirm
that we would only use their information to improve our services, and
would not share the information externally or use it for marketing or junk
mailing.
Now we have created an electronic form on our website which has a box to
tick to confirm all of the same. The form can't be submitted until they
have read a disclaimer and ticked the box. They also have to confirm if
they are the person requesting the adjustment, or are a carer / spouse /
relative etc., and to confirm they are acting with that person's knowledge.
I have confirmed with our Information Security Officer that this is
satisfactory.
As for the staff-accessible database, there are 2 formats - one which is
generally accessible and which doesn't have addresses or other personal
information (just names and requirements) and the other which is password-
protected which has full information. I have full records of who can
access this database.
