"Dragon Speaking Naturally" is very good. I have been using it and
training it for three or four years now and when it works well it can
get about 90% of what I dictate right. Then with careful proofreading to
make sure it has not left the occasional "not" out I can print and send
the document.
I cannot see how any such program is yet ready to replace manual note
taking in the clinical environment.

These are physicians who do not work in a patient environment, but rathe
in a quiet, enclosed space. They are also around for longer than 4
months and have time to get used to the system and build up efficiency.
Can't see how this will inform a decision re use in ED...
