The file is being created by the script in /opt/lcg/var/gip/provider:
-rwxr-xr-x 1 root 1813 Jun 14 12:40 serv03.hep.phy.cam.ac.uk-cache.sh
Remove that script and the cache.ldif once again.
Also remove this file in /opt/lcg/var/gip/ldif:
-rw-r--r-- 1 root 1360 Jul 10 10:41 service-org.glite.ce.ldif

Yes, that is a harmless complaint due to this duplication:

I also see it on a testbed CE, so this probably comes from a YAIM bug.
