The SPM.mat file is a matlab data file. If you are in the directory
that contains the SPM.mat then you can read it from within matlab by typing:
load SPM
This will load the SPM data structure into your matlab workspace into
the variable SPM.
The format of the SPM data structure is documented in the SPM code. The
parts that relate to the design matrix etc are described in
spm_fMRI_design.m (type "help spm_fMRI_design.m" for details).
After you have made the changes you want you can then save it back to
file using:
save SPM SPM
