Rather than explain here, I have created a "Simple axial map analysis"
tutorial which I hope will answer the first two of your questions:

The tutorial shows you how to do the analyses discussed in the handbook.
The third question seems to confuse two types of analyses. There is no
visbility analysis of axial lines: these are already comprise a reduced
form of the visibility structure of the system.
The visibility analysis in Depthmap is applied to grids of systems. You
can of course use a reduced set of points to perform a visibility
analysis (use the pencil rather than the fill tool), which would
effectively perform a line based visibility analysis.
There are various MSc courses which cover the theory in a lot more
detail, and if you are serious about space syntax analysis, you may like
to consider one of these (contact Laura Vaughan [log in to unmask] for
details of UCL's MSc AAS).
