Within my Trust, we only accept IVS results if either they are stamped
with the words "Identified and Validated Sample" or, if not stamped, we
contact the OHS where the sampling was done and ask if their policy was
to check ID prior to sampling. Naturally, if bloods were taken some time
ago, nursing staff couldn't remember whether ID was checked, in which
case we would rebleed.

micro forms, however that is not always the case.
