Do you mean consent for when you take the blood, or for giving the
results? Our department has a very basic consent for taking the bloods,
which consists of two lines, one for the EPP worker to consent for their
blood test, the details of which are not noted (hmm, must rectify this)
and the second line where the OHA verifies the photographic ID.
