I have done a bit more digging re number in caseload-Flint 1993 refers to
'Who's left holding the baby?' (Nuffield institute 1992) and the think tank
that wrote that a caseload of 24-38 would 'radically alter women's exerience
of maternity services' so am going to dig that out-
