My brother and I both received one.
He thought the questions about getting an appointment within two days
were silly. He could imagine an urgent problem that needed sorting out
today, or a routine one that would keep a bit, but not one that had to
be sorted out in two days at most. And he asked "how much is this
costing me?"
I was a bit worried that my questionnaire would be excluded as I
haven't had a personal medical problem for over 12 months. But then I
noticed that the questionnaire's definition of an appointment included
"speaking with a doctor on the phone about any matter". Job done!
