Your recent blood test has shown that one of your kidney function
results is borderline. This is of no cause for immediate concern but as
a precaution your doctor would be grateful if you could provide us with
a urine sample.
* The urine specimen needs to be done in the early morning.
* You need to bring the sample in a small, clean container with a
tight fitting lid.
This must be labelled with:
o Your name
o Your date of birth
o The specimen date
o "eGFR"
* Please return the specimen to the surgery by midday Monday -
Friday.
* You will also need to repeat the blood test in one month's time.
If you have any further queries about this test, please telephone the
surgery and ask to speak to a nurse or your doctor.

Subject: [GP-UK] CKD letter
Tidying up on the CKD element of the QOFand I am about to write to a
few folk requesting that they make an appointment for U&Es and BP
check in preparation for starting on an ACE. Has anyone produced a
good letter explaining this, or know of a good PILs that explains the
whole process?? Obviously many have been CKD3 without them or us
knowing for a time, and if I were a 'lay person' I would wonder what
the hell was going on. No doubt when I have my hypertensive bloods
taken this year I will be told that I am CKD3!
