I may have received some less than objective information from a very
embittered public health practicioner (aka my wife), but apparently the
U.S.' Institute of Medicine has recently released at least a preliminary
report that says that pharmaceutical interventions are the only
evidence-based interventions to mitigate the spread of pandemic
influenza.
Now, I think that most of us have adopted the view that EBH consists of
three important components: external scientific evidence, clinical
expertise, and patient perspective/values. So, in that sense, there may
not be anything wrong with the conclusion that the more public health
oriented strategies (e.g. hand-washing) held so dear among among people
such as my wife just haven't been the subject of much rigorous research.
But, that's where the clinical expertise piece comes in, so there's
every good reason to continue those strategies if they are consistent
with the clinician's conceptual model of how influenza spreads, and if
their experience has led them to believe that they have been effective
in the past. Apparently (allegedly?), however, the IOM report was
written in such a way as to enflame many folks.
So, not sure if that's the type of thing for which you were looking,
Jon, but it's receiving a lot of interest over here. Sorry I don't have
time to find an exact citation to support all of these
non-evidence-based assertions I've been making about this report!

Rob

Subject: Research practice gap
Hi!
Does anyone have any good examples of where clinical trials/systematic
reviews have published results which simply don't translate into
everyday practice?
Best wishes
jon
