You're right that the nomenclature if confusing, but the scene is said
to depict the Conception of Christ (rather than any other stage of his
pre-birth life); from memory there are a four or five other
representations of the subject known, all English, all C15, and all
iconographically pretty comparable: whether it occurs elsewhere in
Christian art I know not.
If the lightning were accompanied by thunder, at least it seems likely
that the earth moved...

Do you mean the Annunciation (or possibly the Incarnation)? The
Conception
of the Virgin is Mary herself being conceived, in a (relatively) more
conventional (but still Immaculate) way. (I am a bit hazy, as I suspect
they were in the Middle Ages, as to whether Mary's Immaculate Conception
included its mechanism as well as its nature...)
